Important information about our practice policies and procedures.
Here at Spencer Road Family Practice we understand that circumstances change, however as we are a busy practice we would appreciate at least two (2) hours notice if patients are unable to attend their appointment.
Please phone us on 9493 8333 to cancel or reschedule the appointment.
If the appointment has been made online via HotDoc, the appointment can be cancelled through your log in to their website or app.
This allows us to help other patients who may be in need of medical attention on the day.
Please be aware that it is practice policy to charge a non-attendance fee of $55.00, which is non-refundable from Medicare, if there are more than 3 missed appointments within a 12 month period.
Our practice engages the services of the Western Australian Deputising Medical Service (WADMS), who can provide after hours care and home visits when we are not available.
If you require this service please call one of our friendly receptionists who can arrange this for you.
If the practice is closed, you can call WADMS directly on 9321 9133.
For a GP After Hours, the closest is located at the entrance of the Armadale Health Service, 3056 Albany Highway, Mount Nasura 6112. Ph: 9391 2285. Open from 7pm.
Alternatively you can phone Health Direct for advice on 1800 022 222.
In case of an emergency, call 000.
If you require a Medical Certificate please ask the doctor during the consultation. Medical Certificates are a legal document. Therefore, doctors are unable to back-date certificates, or provide them without consultation.
If you have already been provided with a medical certificate and you need the dates extended, an appointment with the doctor will be required.
Our practice has a recall system for health promotion, prevention and assisting the management of chronic disease. Please advise the receptionist if you wish to have your name removed from this program.
We have switched to environmentally friendly and secure mobile messages (SMS) via HotDoc instead of letters to communicate health messages from doctors to patients.
You will receive an SMS with a unique and secure link. You will need to verify your last name and date of birth to read your recall health message.
If your doctor sends you for routine tests, ideally an appointment should be made with your doctor to discuss all test results. If your test gives an abnormal result, one of our staff members will contact you for an appointment.
Please note that it is practice policy that test results will not be given out over the phone.
Doctors can be contacted whilst rostered on during practice opening hours. If the doctor is busy, our receptionist will take your details and the nature of your problem and forward to the doctor, who will return your call as soon as possible.
In the case of an emergency, your call will be put through to one of our nurses.
If you have been seen face to face in the last 12 months you may be eligible for a Telephone Consult. Please discuss this with reception on 9493 8333.
This privacy policy provides information on how your personal information (including health information) is collected and used within our practice, and the circumstances in which we may share it with third parties.
When you register as a patient at Spencer Road Family Practice, you provide consent for our GPs and Practice staff to access and use your personal information so they can provide you with the best possible healthcare. Only staff who need to see your personal information will have access to it.
The information we collect includes: names, date of birth, addresses, contact details, medical information including medical history, medications, allergies and adverse events, immunisations, social history, family history and risk factors, Medicare number, health care identifiers, and health fund details.
You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are required by law to only deal with identified individuals.
We sometimes share your personal information with third parties who work with our practice for business purposes (such as accreditation agencies or IT providers), with other healthcare providers, when required or authorised by law, or when necessary to lessen or prevent a serious threat to health or safety.
Spencer Road Family Practice will not use your personal information for marketing any of our goods or services directly to you without your express consent.
You have the right to request access to, and correction of, your personal information. Please put requests in writing and our practice will respond within 30 days.
